About Chris...
I founded Care Connections based on a lifelong passion for serving people. I am a licensed Physical Therapist with over 15 years of physical therapy experience, much of which involved treating the elderly. I have worked in multiple medical arenas including hospitals, rehabilitation facilities, home health agencies, and nursing homes.

In addition, I am a Certified Case Manager and a certified member of the Aging Life Care Association. Becoming an advanced level member of this professional organization requires years of experience, certification by one of four credentialing bodies, professional references, and a baccalaureate or higher degree in a field related to caring for seniors. About Lisa...

I'm a registered nurse with four years of experience in a rehabilitation hospital where I had the opportunity to provide compassionate care for patients with a wide range of diagnoses. This was a great learning experience which emphasized the importance of teamwork and an interdisciplinary approach to care. I have an Associate’s degree in Nursing from Cleveland State Community College and a Bachelor’s degree in Sociology from Appalachian State University. I look forward to applying my knowledge and experience in order to advocate for clients and to help them obtain exceptional care.
